CASE MANAGEMENT

The Case Management Department’s philosophy stresses that individuals with disabilities should lead active and productive lives while getting the services that they need to achieve their goals. The department has skilled advocates in managing the complex system of public entitlements and legal mandates. Understandably, parents and individuals with disabilities can become overwhelmed with the challenges they face. Our Agency’s Case Management Department assists individuals and families in meeting those complex needs.

Individuals receiving case management services receive referral information, advocacy, at-home visits and assistance in life planning. The department has grown and now services children and their families as well as adults in the community.

The Case Management Department also oversees a supported employment program which assists individuals with developmental disabilities who have the desire and motivation to seek and retain competitive employment. The department specializes in those individuals with multiple disabilities.

The Case Management Department also provides services under the Home Service Program which is partially funded by the New York State Elks. This program offers free developmental screenings for children under five years to assist in the early identification of children with disabilities. Other services are also provided to children and adults through the Home Service Program.
